A single program is one show or episode to be aired on DCTV. (We may air it multiple times per month.)

Please use a different form if you would like to submit one of the following:

Mini Series (6-8 episodes)
Series Program (16 episodes)
Public Service Announcement (PSA)

All Producers submitting programs must watch the Orientation Video and understand the contents, which you have to agree to on the following form.
Note: Producers have to have watched the video and agree to terms - but do not have to watch it every time they submit a program.

I understand and agree, in accordance with DCTV Producer Manual, that by requesting this program to be telecast, I am (or the member organization I represent is) an access channel producer, and therefore fully responsible for content of the program when tele cast, whether or not I actually produced the program myself. I am (or the member organization I represent is) fully liable for all claims arising from unauthorized use of copyrighted material or any other charges of unlawful conduct an will hold harmless the Public Access Corporation of DC (DCTV), its directors, officers and staff. I have obtained the necessary releases to have this program telecast. I further acknowledge that I have received all rights and clearances for this program.
